How to receive your rebate (Up to $12.00):

1. Purchase: Up to 3 gallons of Thompson's® WaterSeal® Advanced Wood Protector in a single purchase OR up to 4 gallons of Thompson's® WaterSeal® Waterproofer PLUS Wood Protector in a single purchase at a participating retailer from 5/22/09 – 7/5/09.
2. Print out and complete this form.
3. Send the following items to the address below:
   • A) This form filled out completely;
   • B) The original cash register receipt (dated between 5/22/09 –7/5/09) with the purchase price(s) circled;
   • C) The original UPC codes from the label of each container as proof of purchase. For metal cans write the UPC numbers on a separate sheet of paper.
4. Mail items A, B, and C to:

5. Receive by Mail: A rebate in the amount of either $3.00 or $4.00 for every gallon purchased depending on products purchased. Maximum amount is $12.00.
